I've had both impact on file attachments.

If the attachment's source machine were recently upgraded or audited, the attaching machine could've lost the authorization to read the file.

Also, network issues will also cause this problem.

If there's a third-party application that's establishing the share, that can also be a factor. As files grow in size, third-party software sometimes reveals cracks in its (or the underlying OS's) ability to drag huge blocks of data across the network.

If the file is really huge, you might also run out of server memory. I don't think you'd have a problem in the hundred-MB range, though.
